  Observation. Regarding intake December 8, 2025. Through the investigation process, Licensing determined an incident of missing and wandering occurred on December 5, 2025. This incident was not reported to licensing within 24 hours.

  Corrective action. Ensure to notify Licensing within 24 hours after a child was involved in, or may have been involved in, a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On December 19, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: Licensee Contact will review licensing requirements for reportable incidents with all staff on January 16, 2026, at a Professional Development Day. The Licensee Contact will follow up with staff not in attendance by January 23, 2026. This corrective action plan was reviewed and originally accepted by Licensing. Licensee changed and updated the corrective action plan to include all staff would review Reportable Incident protocols at the February 17, 2026, staff meeting. Licensing reviewed and accepted the updated corrective action plan on January 12, 2026.

• A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure

  Observation. Regarding intake dated December 8, 2025. Through the investigation process, Licensing determined that for the before school program children's attendance records showed the time of arrival but not the time of departure.

  Corrective action. Ensure a record is kept for all children showing the time of arrival and the time of departure. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On December 19, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included new attendance records had been created to show time of arrival and time of departure in both the morning and afternoon school age programs. This corrective action plan was reviewed and originally accepted by Licensing. Licensee changed and updated the corrective action plan, to include all staff would review the requirements for attendance records at the February 17, 2026, staff meeting. Licensing reviewed and accepted the updated corrective action plan on January 12, 2026.
