Observation. An area of trellis is missing from the fencing in the yard and one part of the trellis is loose.
Corrective action. The manager said this has been reported and she is waiting for repairs to be made.

Observation. One edge of a carpet in the nap room is showing signs of wear and tear and is frayed.
Corrective action. Please ensure the frayed edge of the carpet is repaired. The use of tape was discussed with the manager.

Observation. Files for 26 children were reviewed. 6 files did not contain the name and contact information for a doctor or medical clinic.
Corrective action. Please ensure all files contain the required information.
