    Observation. A staff used inappropriate behavioural guidance strategies when she created a video recording of a child to document his behaviour and repeated a command to him multiple times.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Upon inspection, licensing observed the following: Outdoor: - Rusted metal dump truck toy. Rust can cause a potential health hazard. (Repeat contravention) - Spider webs on the play structure - Loosen wooden panel on the play structure - Rusted nails sticking out on the play structure Indoor: - Multiple dry wall paints chipped throughout the facility.

    Corrective action. The Staff said she will ask the Director to ensure above identified items to be either replaced or fixed.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

    Observation. Upon inspection, licensing observed the following: Indoor: - Window cleaner solutions were stored in the bathtub, accessible to the children - Toilet cleaning solution was stored under the washroom sink. A lock was observed but it could not be lock properly. It was not secured and the cupboard was easy to open. This was a REPEAT contravention from report CLAN-CSETMK - A door to a storage room in the main cubby area was easy to open: loose cord and vacuum were stored in there. They were accessible to children. Loose cord poses a potential strangulation hazard to children. Outdoor: A shed in the outdoor play area was not locked and accessible to children. In the shed, there were large lawn mower machine, 3 mosquito flying insect spray, garden tools, ice melts, and few large items observed. These pose potential hazard to children.

    Corrective action. The Staff said she will ask the Director to address the identified items by making them inaccessible to children.

  • STAFFING

    A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

    Observation. Upon review the staff's file, the staff's ECE certificate was not validated by using the system maintained by the early childhood educator registry.

    Corrective action. The Staff said she will ask the Director to validate her certificate.

    Corrective action. The Staff said she will ask the Director to submit a manager change request to the licensing.

    Observation. Outdoor play space: - metal black gate with no lock leading to the driveway - a gap by the neighboring fence, posing entrapment hazard Washroom: - shaving cream was stored in the bathtub, accessible to the children

    Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care

Observation (CORRECTED DURING INSPECTION): 8 children were present; only 6 children were signed in.

    Corrective action. Ensure that children's attendance is recorded accurately.

    A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

    Observation. Licensing observed the following inside outside the facility: -Two bottles of bleach were observed under the sink. A lock was applied but it was not secured and the cupboard was easy to open -Rust was observed on the climber. This is a repeat contravention from inspection #CLAN-C5E24K -Rust was observed on the outdoor metal dump truck toy. Rust can cause a potential health hazard.

    Corrective action. Please ensure the children do not have access to any objects or substances that may be hazardous to their health and safety.

    The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.

    Observation. The care plan was not reviewed annually with the parent or any person requested to by the parent.

    Corrective action. Please ensure to review the care plan with the parent on an annual basis. Licensing recommends that the care plan is posted in the facility.

    Observation. Three children's records were reviewed. One child in attendance did not have a record.

    Corrective action. Please ensure the children's records are completed prior to the child's first day and that this record is on site and available for review.

    A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

    Observation. Licensing observed the staff records. The stole staff working in the facility did not have a criminal record check (CRC) on file. In discussion with the Licensee she stated she had applied for a CRC for the staff member two weeks ago but has not received it. The health and safety risk to children was discussed.

    Corrective action. Please ensure a cleared criminal record check is completed with the Criminal Record Review Board of British Columbia prior to an employees first day of work at the facility. A written supervision plan is required to be submitted to Licensing outlining how the employee will be supervised until the CRC is completed.

    A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

    Observation. During the inspection the following was observed: -A rusty scooter was located both inside and outside the fence. Rust can present a potential health hazard. -A bottle of concentrated bleach was located under the sink. The door to the cupboard containing the bleach had a child safety lock but this was easy to open -Hand sanitizer was within easy reach.

    Corrective action. Please ensure children do not have access to any objects or substances that are hazardous to their health and safety.

    A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,

    Observation. A child's record was reviewed. In the record it stated the child had serious allergies and required emergency medication in the event of an exposure. In discussion with the Manager she was not aware of this. The Manager will follow up with the child's family.

    Corrective action. Please ensure that the children's records are reviewed and if necessary, emergency medication and care plans are put into place to protect the safety of the children in care.

    Observation. Licensing reviewed the tag attached to the fire extinguisher. The fire extinguisher was due for servicing on February 11, 2022.

    Corrective action. Please ensure the fire extinguisher is serviced prior to the expiry date to ensure you are prepared in an emergency.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

    Observation. Licensing observed the following inside the facility: -Plant food chemical was accessible to the children -Disinfectant spray in he washroom was located at the children's level -two chairs in the kitchen were placed in the corner and abutted two intersecting windows. The windows were open greater than four inches wide and no screens, locks or limiters were applied. This is a serious health and safety risk to the children in care as these windows are located on the second floor. Licensing strongly recommended reducing the window openings to under 4 feet and applying window limiters and locks when windows are open. Having windows at the childrens level that were not securely locked is a repeat contravention from inspection # CLAN-BFENMC Licensing observed the following outside the facility -A metal bench was located right next to and underneath the climbing structure. The bench could create a hazard as a six foot distance is required around the climber. -A metal bench had rust in several spots. This could present a potential health hazard.

    Corrective action. Please ensure children do not have access to any objects or substances that are harmful to the children's health and safety.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. During the inspection licensing observed that the windows in the play area were left unlocked. A lock was present but not applied. This could present a potential falling hazard.

    Corrective action. Please ensure a healthy and safe environment is provided at all times.

    A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.

    Observation. Licensing observed that both the outdoor playground gates were locked with bungy cords while the children were playing in the outdoor play area. In discussion with the staff she stated that the children open the gates. We discussed active supervision and purchasing a lock that is accessible to staff but not children.

    Corrective action. Please ensure an emergency exit is not blocked to ensure a safe exit in a potential emergency.

    A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

    Observation. During the inspection licensing asked where the emergency supplies were located. A small bag with minimal supplies was observed. No food and water was available in case of emergency. This is a repeat contravention from inspection #CLAN-BHXQ3E

    Corrective action. Please ensure there are sufficient emergency supplies to prepare for, mitigate, respond and recover in an emergency. Licensing recommends sufficient supplies for a 72 hour period.

    Observation. During the inspection licensing observed the attendance records. Three departure times were missing for the month of July.

    Corrective action. Please ensure the attendance records are complete including the arrival and departure times of the children in care.

    Observation. Licensing reviewed the children's files. One child had a baby picture on file although the child was older than 2.5 years. This is a repeat contravention from inspection #CLAN-BNYVBK

    Corrective action. Please ensure a child's photo is current to ensure it is an accurate representation of the child. This may be required in case of emergency.

    Despite the employee qualifications required by Column 4 of the table in section 1 of Schedule E [group sizes and employee to children ratios], a licensee must replace an absent employee described in Column 1 of the following table who is absent (b) for 30 consecutive days or more with a person described in Column 3 opposite that employee and the row in Column 2 labelled as "≥ 30 days".

    Observation. Licensing observed that the Manager is no longer working at the facility. Another Manager's name was listed but no information has been received by licensing to process the change in Manager.

    Corrective action. Please ensure a new Manager is appointed after being absent for more than 30 days. Notice of this change must be given to licensing to comply with the legislation.

    Observation. Licensing reviewed the children's pictures at the inspection. The children's pictures were taken when they were very young. The two children present during the inspection are currently over three years old.

    Corrective action. Please ensure the children's pictures are current so they are able to be used to readily identify a child in an emergency.

    A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.

    Observation. During the inspection the staff records were reviewed. One staff member/Director, who was the sole employee at the facility during the inspection, had an expired first aid certificate from December 2019. In discussion with the staff member she stated she has already taken the first aid course. The staff member showed documentation of her payment and stated she took the first aid course in December. The certificate was not present at the facility as she has not received it..

    Corrective action. Please ensure all staff have a valid first aid certificate to ensure children have access to an employee who can provide first aid or CPR in an emergency.
