Observation. The metal fence that encloses the Seedlings <36 months outdoor play space has worn over 6 years of use. This fencing slats are breaking at the bottom. Today's inspection revealed two slats tapped together. The tape is able to be moved and this could create a head entrapment. Seedlings playground: has a smallarea of Astro turf that has areas of tripping potential because of old surfacing/tree roots. Staff also report water pools in this area. Observed installation of a vinyl covered rope with rings and a trapeze type swinging piece attached. The rope has been secured with a winch so it is secured. As discussed the height of the equipment does not exceed 3 ft so there is not a requirement for resilient ground surfacing under. There is a wooden barrier at side of the trapeze that could be a fall hazard. Saplings playground: a wooden board is missing from the (Family Place) building exposing building material that children should not access. The sensory pathway has exposed tripping hazards as the landscaping materials planted have not grown. The wooden climbing/slide piece that is outside has broken areas as the slide as broken off.

Observation. The metal fence that encloses the <36 months outdoor play space has worn over 6 years of use. This fencing goes from grade level to a patio point that has more than a 10 foot fall zone. At today's inspection the staff showed the LO how the metal is aging and the risk of injury to a child if the metal bends. Staff reported that a child ran into the fence with some force and the metal was observed to move/bend. After this Incident the Licensee installed metal bracing along the horizontal slats as an additional safety measure..
