  Observation. Licensing observed the door mat by the upstairs entrance has started to curl up, which is creating a potential tripping hazard.

  Observation. Licensing reviewed a selection of children’s records and found the parental consent permitting release of the child to someone other than the parent were not kept.

  Observation. Licensing observed the facility licence located in the lower level Preschool classroom in an area dedicated for the Educators desk work.
