• C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical

  Observation. Licensing was not notified of a large indoor permanent play structure being assembled in the facilities gym. Licensing must be informed of any floor plan changes that are occurring at the facility. Submit to licensing by August 7, 2026, a floor plan change notification that includes the measurements of the permanent play structure.

• C1.2 - Does the licensee operate the facility in a manner that will promote the health, safety and dignity of persons in care?Non-critical

  Observation. Radon results have not been submitted to licensing. Unknown level of radon may increase risk to the health and safety of children. Submit to licensing by January 31, 2027, test results from a long term radon test. The submitted lead in drinking water results are incomplete and must be resubmitted to licensing in their entirety. Unknown results for lead content in drinking water may increase risk to the health and safety of children. Submit to licensing by January 31, 2027, results from a test for lead in drinking water.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. Staff files are incomplete, one file is missing a criminal record clearance letter, another file is missing verification of the educators certificate. Incomplete staff files increase the risk that individuals who are not properly screened or trained may be providing care to children. Submit to licensing by August 6, 2026, evidence the missing items have been obtained and are in the staff files.

• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. The facilities fire extinguishers are unsecured on the floor and are accessible to children. Unsecured fire extinguishers that are accessible to children present risk of injury, accidental discharge and chemical exposure. Submit to licensing by August 7, 2026, evidence the fire extinguishers have been properly mounted and secured. There is a variety of construction material and a ladder in the hallway located by the gym. The items in the hallway may increase risk of children tripping or climbing on unsafe materials. Submit to licensing by August 7, 2026, evidence the hallway has been cleared of debris. There is no method of ensuring food safe temperatures in the Group Child Care (under 36 months) infant room refrigerator. The absence of a refrigerator thermometer creates a food safety risk because staff cannot verify that food is being stored at a safe temperature. Submit to licensing by August 7, 2026, evidence the fridge contains a thermometer.
