  Observation. Licensing observed the following: - in the indoor play areas, several toy furniture and supplies hampering exits in the case of fire or other emergency. - in the outdoor play areas, there are falling and tripping hazards such as various sized wood slabs, broken riding toys and wooden doll house pieces leaning against the house.

  Observation. Licensing observed the following: - in the outdoor play areas, the north west corner of the fence is missing. - the large gap in the fence is accessible to children. - the second outdoor play area is not enclosed.

  Observation. Licensing observed the absence of emergency equipment such as flashlight and radio from the emergency evacuation kit.
