  Observation. A child in care had access to a knife at the facility.

  Corrective action. The knife has been relocated to a place that is inaccessible to children.

  Observation. A staff required children to use the washroom prior to going to the park, and used physical guidance in the process.

• RECORDS AND REPORTING

  A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.

  Observation. Manager did not notify the licensing officer within 24 hours that a child was involved in or may have been involved in a reportable incident as described in Schedule H as required.

  Corrective action. Ensure 24 hour notification is met if a child was involved in or may have been involved in a reportable incident as described in schedule H.
