Observation. On July 08, 2026, there was no record confirming that all employees had practiced implementing the emergency plan within the past year. There continues to be no record confirming that the emergency plan has been practiced within the last 12 months. A record must be maintained indicating that the emergency plan developed by the Licensee has been practiced by all employees at least once annually. Observation. Radon test results were provided on July 17, 2026; however, the testing was completed outside the required testing period of November through April. Radon is a naturally occurring radioactive gas that can accumulate indoors and pose a risk to health with prolonged exposure. To help ensure a safe environment, radon testing must be conducted during the cooler months of November through April using a long-term test of at least 91 days. - C4.1 - Are facility records current and complete?Non-critical
Observation. One child's record is missing the name and phone number of an emergency contact. Ensure all required emergency contact information is collected prior to a child being provided with care. Submit written confirmation by July 17, 2026, that the missing information has been added to the child's records. A child's medication record does not include the dosage of medication given. When medication is provided to a child, the dosage given must be recorded to ensure accurate documentation. Submit written confirmation by July 17, 2026, confirming medication records include the dosage given. There are no care plans in place for children with asthma, allergies, or other conditions requiring extra support. To ensure the health, safety, and well-being of children requiring extra support, a detailed care plan must be developed in consultation with the child's parent(s) and any other person requested by the parent(s). Submit written confirmation by July 17, 2026, confirming that all children who require life-saving medication or additional support have an individualized care plan in place. Two employee criminal record checks could not be produced for inspection. To verify that employees have valid criminal record checks, these records must be maintained and readily available for review at all times. Submit written confirmation by July 17, 2026, confirming that current criminal record checks are on file and available for all employees as required.

- C1.2 - Does the licensee operate the facility in a manner that will promote the health, safety and dignity of persons in care?Non-critical

Submit written confirmation by July 17, 2026, confirming that all medications are current and not expired, and are safe and effective for use.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
