Observation. It is alleged that a staff member became upset with another staff member and began yelling at them, in the presence of a child, which made the child feel afraid and upset. Follow up to this complaint included staff interviews, review of facility documentation and footage from a motion censored security camera. As a result of statements made in staff interviews, a review of security camera footage and facility documentation, it is determined that the nature of the communication between the staff members created a negative environment in the presence of the child, which may have resulted in the child feeling afraid or upset. Therefore this allegation is substantiated. Submit a compliance plan to address this issue on or before September 25, 2026.

Observation. Of the child registrations that were reviewed, the following items were missing: 4 records were missing emergency contact information. This must be someone other than a parent. 2 records were missing medical practitioner information. It is important to have this information on file in case of emergency. Submit a compliance plan to address this issue on or before November 13, 2025.
